@DrDiGiorgio In Belgium almost all medical imaging is put in a cloud (usually PACSonWeb). You just need the access code on the radiology report (or access as a care giver to his/her online medical files) .

In case people are wondering why technology hasn’t solved this: The problem is the hospital that did the first MRI. They don’t want to share. Sharing data empowers their competitors so they create as much friction as possible. Independent imaging centers make access easy by uploading imaging to the cloud. Their business model revolves around convenience, transparent prices, and customer service. A large hospital system wants to keep patients within the system. “Leakage,” as they call it when patients leave the system, is to be avoided. The technology exists. Hospitals just don’t want to use it because they want to make it difficult for you, the patient, to seek care elsewhere.

Congrats to study authors. Sadly it appears RP didn’t result in delayed progression in low volume M1, but earlier calling of failure appeared to lead to earlier initiation of life prolonging systemic therapy which improves CSM. Unfortunate RP couldn’t be shown to delay progression. Even the overall stampede cohort including high volume showed FFS benefit of RT to primary. Good to have the data out, especially the common high grade toxicity, much higher than expected with RP in localized dz. Look forward to SWOG trial and curious what percent will be low volume receiving RP. It may be better powered to assess FFS benefit.

That first pillar should probably be "new treatments". But, you know.
