In thoracic pathology, metastases are frequently encountered. It is essential to be familiar and comfortable with interpreting immunostains in lung tumors. Breast carcinomas often express neuroendocrine markers without being neuroendocrine tumors (e.g., large cell neuroendocrine carcinoma). A lung biopsy showing carcinoma strongly positive for neuroendocrine markers is not necessarily a primary or metastatic neuroendocrine carcinoma, especially with a history of breast cancer. The case below illustrates this point. Always fully work up malignant lung biopsies, particularly with a history of malignancy. This tumor was also ER- and PR-positive, consistent with the clinical history (invasive breast carcinomas with neuroendocrine features are often ER- and PR-positive and HER2-negative, but I am not a breast pathologist!). Gastric neuroendocrine tumors-Previous post corrected Size cutoff: Lesions <0.5 mm are often considered tumorlets or precursor lesions, while those ≥0.5 mm are classified as neuroendocrine tumors (approximately 1 high-power field). Do we grade thyroid cancer? Yes. Poorly differentiated carcinomas: Histologically poorly differentiated tumor with solid, trabecular, or insular pattern of growth with - Convoluted nuclei, or - ≥3 mitoses per 2 mm², or - Tumor necrosis If you have solid, trabecular, or insular pattern of growth and nuclear atypia of PTC, you call these PTC, solid subtype.
