Costs associated with anastomotic leak after left-sided colorectal surgery: retrospective real-world study in England ➡️doi.org/10.1093/bjsope… This retrospective matched cohort study compared healthcare resource utilization and costs between patients with and without anastomotic leak (AL) after left-sided colorectal surgery to highlight the significant burden of AL and to address the evidence gap regarding its economic impact in England. Negative-pressure wound therapy in thoracic and abdominal surgery: meta-analysis of randomized trials ➡️doi.org/10.1093/bjsope… This systematic review and meta-analysis of 45 randomized trials including over 12 000 patients found that closed-incision negative-pressure wound therapy (NPWT) significantly reduces surgical site infections (SSIs) and shortens hospital stay following open abdominal surgery, with consistent effects across device types. No benefit was observed for organ/space infection, fascial dehiscence or reoperation, and evidence of publication bias suggests that, although the magnitude of benefit may be overestimated, the reduction in SSI remains statistically significant when adjusted for this. Transanal irrigation for low anterior resection syndrome treatment: international multicentre randomized clinical trial ➡️doi.org/10.1093/bjsope… In this multicentre randomized feasibility trial, transanal irrigation (TAI) was evaluated in patients with major low anterior resection syndrome persisting at least 12 months after anterior resection. Compared with best supportive care, TAI significantly improved bowel function and quality of life at 12 months, with markedly lower LARS and Wexner incontinence scores and higher bowel function scores. Treatment adherence was high, with 75% of patients continuing daily irrigation, and patient satisfaction consistently favoured TAI. Quality metrics for perioperative red blood cell transfusion in gastrointestinal cancer surgery: international Delphi consensus ➡️doi.org/10.1093/bjsope… A consensus on quality metrics to assess and monitor the use of perioperative red blood cell transfusion (RBCT) in gastrointestinal cancer surgery was generated through a four-round modified Delphi process with 37 international experts. Clinical Outcomes Associated with Prior GLP-1 Exposure in Burned Patients ➡️doi.org/10.1093/bjs/zn… 💉 GLP-1 receptor agonists may offer benefits beyond glycaemic control in burn patients, with potential anti-inflammatory and immunomodulatory effects. 📊 In a large matched cohort (n=8,307 per group), prior GLP-1 use was associated with 54% lower mortality (OR 0.46; p<0.001). 🏥 Significant reductions were seen in ICU admission (−35%) and intubation (−62%), indicating lower critical care needs. 🦠 Infectious complications were also reduced, including sepsis (−32%), pneumonia (−24%), and MRSA (−27%), with less antibiotic use. ⚠️ A signal toward increased hypertrophic scarring (+27%) was observed at longer follow-up. 🎯 Prior GLP-1 exposure may improve burn outcomes, supporting further prospective studies to guide perioperative strategies. Differences in recovery time between trapeziectomy and carpometacarpal joint replacement: meta-analysis ➡️ doi.org/10.1093/bjsope… This study aimed to define and compare patient-reported recovery trajectories following trapeziectomy and carpometacarpal (CMC) joint replacement for thumb-base osteoarthritis. This study provides level II evidence that CMC joint replacement results in faster recovery than trapeziectomy over the first 3 months after surgery, with newer dual-mobility implants associated with quicker recovery in DASH and key pinch strength than single-mobility implants.
