@RobertMSterling My daughter used to say ‘flingo bird’ for flamingoes and ‘magenda’ for ‘agenda’ as in ‘ what’s our magenda for the day’ and we still use these words 20 years later

The best random part about having small kids is how they mispronounce things and it’s so adorable that you not only don’t correct them, you start adopting it yourself. During Covid, our daughter couldn’t say “hand sanitizer” correctly. Three years later and we all still call it “hanzatizer.” When my son was two, he had a toy Elmo guitar that he called his “kintar.” My wife and I only stopped pronouncing it like that once he started real kintar lessons and we sounded dumb in front of the teacher. Our toddler is two and she calls her backpack a “pack pack.” Now the other kids talk about getting their pack packs ready for school every day. We’re having pasghetti for dinner tonight. It probably sounds stupid, but it’s little things like this that make parenthood—despite all the challenges—an irreplaceable joy.

@acweyand 💯. Also applies to my perimenopausal patients w heavy periods who think it is always just hormones when their fatigue and hair loss are often linked to low ferritin. Ferritin should be a part of a ‘peri’ panel.

@doctorORbust 100%! I am now seeing many women being referred for menopause and while I LOVE menopause care and we need to improve it - it can’t be the assumed primary everything bucket for every >40 woman with fatigue. I pick up a TON of b12, d, ferritin deficiency. Basic you would think

I think it’s weird when a patient is sent to for supposed fibromyalgia and fatigue and I’m the first to send an iron and ferritin. Her ferritin was 7. I was like, do you like eating ice? She devours it. Please send Fe + ferritin and remember fibromyalgia is a Dx of exclusion.

@LukeEpplin Not sure how this great story ended up in my feed but… I delivered babies for 15 years at a certain uptown hospital where labor and delivery is 2 blocks down from the ER… we all had a tender place in our hearts for the guys who fainted. Congrats hope the whole fam is well!

@hubermanlab For ‘reg’ MDs - there is fear of controversy and shaming, of cancellation and loss of credibility , career trajectory and income. And given our malpractice climate - even if there is patient harm - as long as the treatment was ‘guideline based’ - liability is lower.

So interesting that whenever I put out the call for “suggest expert on topic X”, the suggestions converge for 1) standard MD, PhDs 2) “alternative” MDs/practitioners …and yet when I compare the suggestions made by these two seemingly disparate groups MOST of what they suggest lines up… yet they ask me not to share that they personally agree with the other group; it’s amusing, and a bit scary to be honest. Are we really that afraid to say what we think about best health practices?

@Theholisticpsyc As a gynecologist specializing in midlife health and menopause, this is rampant and infuriating. The link between hormones and mental health is real. so many midlife women are handed an antidepressant prescription without ever considering hormonal root causes!

I think about how people are in therapy dealing with hormonal imbalances, thyroid issues, or chronic sympathetic activation. All of these present in changes of mood. How many people are diagnosed as mentally ill without acknowledging what’s going on in their physical body.

@jfitzgeraldMD And while you are in meeting #12 with clients 13-16 waiting impatiently, you get a text from your husband reminding you that he is traveling today and now you also need to figure who will let the dog out mid day or you may have to deal w unpleasant consequences after clinic

A non-medical friend recently asked me to describe clinic. I told her to imagine you have 20 meetings in a day, half of them new clients with urgent needs. Each requires your best self. You are late for at least 10 of them. You must prepare a report and deliverables for each one.

@adamcifu Yes!!! Or in my world ‘cervical cancer screening’. Also when ‘pap smear’ is added to the surgical procedure list.

An admittedly unimportant pet peeve. Not every "visit diagnosis" need be added to the "problem list." Ok, I feel better now.

@drherrling I am blown away by some of the answers here. Do people not realize that other than the most simple thing for a patient we know, not much can be accomplished in 6-10 min? We are not robots and 6-10 min visits eventually lead to bad care or burnout.

I am a family doctor in BC. I see 30-40 patients per day (no lunch or coffee breaks, maybe time for a quick run to the bathroom), followed by a few hours of paperwork - charting, lab results, imaging reports, consults, insurance forms, pharmacy queries, urgent calls, etc.

@SahilBloom As an obgyn, I think 24 hour shifts as a resident are different than 24 hour shifts in your let’s say 40s, not that I’m a fan of either. I’m 48 now and stopped ob when I noticed just how much more lack of sleep affected my performance, health, and family the older I got.

How is it that doctors are allowed to work 24-hour shifts? We restrict the number of hours that a truck driver can drive for safety reasons, but a doctor can work around the clock. What am I missing?

@FitFounder After a few frustrating months of charts/rewards with my daughter, I decided to go analytical. Did my research and found a book from the 70s called ‘how to potty train your child in less than a day.’ One day off of work for me and FOUR HOURS later she was potty trained.

@gbriddick @2HeartsWellness @DrJenGunter @Neurofourier I experienced a severe rxn to Moderna. Consulted immunology - no answers but advised reporting to vaers. Reported to moderna, vsafe and VAERS, no f/u from any. Tbh, didn’t know re: CISA - but it does not seem like a reporting database but rather a research/consultation tool?

Abusing VAERS should get you reported to your medical specialty board. Just saying.
