Marriage certificates are *not* included, effectively disenfranchising many of the 69M women whose last names don’t match their birth certificates (not an issue for those who buy expensive passports or have “Enhanced Drivers Licenses” that do match) 1/

What it’s like to be in healthcare right now: “We will pay you $2 for that pie” “But it costs me $4 in pie ingredients and $2 in human labor to make a pie. We need $6 to break even” “Sorry, we’ll only pay you $2 for that pie. We suggest you make many more pies that you also sell for $2/pie. New requirement is 6 pies/day” “But then we are still losing $4 per pie. How does making more pies help us?” “We just found out flour is on shortage so you’ll have to buy more expensive imported flour and will lose $5 per pie. Or you can consider making pies without flour.” “But I can’t make pies if we keep losing money on pies. And I can’t make pies without flour” “We suggest you ditch rolling pins and remove any fruit filling as a cost saving measure. And consider limiting how much time you bake the pie for to use less oven time” “But then how can I even make a pie that resembles a pie?” “We appreciate the question and have no response. We empower YOU to solve this problem. We have scheduled 4 meetings to discuss how you can make more pies with less ingredients and less oven time and less labor” “But it seems the root of the problem here is actually that we don’t get reimbursed fairly for our pies…” “Don’t say that out loud. Proposed Conversion Factor of $32.26. in 2025. It was $36.69 in 1998. So even w/o inflation adjustment, doctors in 1998 made more from Medicare than doctors in 2025. Adjusted for inflation, it should be $71.33 ($32.26 in 1998 would be $17.05) #MedTwitter

Breaking: CMS has again proposed a negative Medicare physician fee schedule update for 2025 with dangerous implications for beneficiary access to care. A 2.8% reduction to the conversion factor would be alarming in the best circumstances, but to propose doing so at a time when 92% of medical groups report increased operating costs and are otherwise struggling to remain financially viable is critically short-sighted. Medicare physician reimbursement is on a dire trajectory and these ongoing cuts continue to undermine the ability of medical practices to keep their doors open and function effectively — the need for comprehensive reform is paramount. @MGMA is once again calling on Congress to pass the Strengthening Medicare for Patients and Providers Act to implement an annual inflation-based physician payment update tied to the Medicare Economic Index, and modernize Medicare’s antiquated budget neutrality policies by enacting the Provider Reimbursement Stability Act.
