The fact that the study was funded and interpreted by ABIM personnel was clearly disclosed by the authors in both the conflict of interest sections. If you believes there are still issues with transparency, it is necessary to you to provide your evidences. In their study, their interpretation of the relationship between physicians in the top vs. bottom LKA quartiles and patient outcomes is entirely reasonable within the limit of the pseudo-randomized design and assumption of statistics model that they chose. And all are concisely mentioned in the limitations in the discussion section. The important thing for you and other readers is that the authors have no attempt to draw conclusions beyond the boundaries of the model or study design that they constructed and therefore I think they did not have any intention to “scam” anyone.

Although the study has certain limitations in design and methodology, these were openly mentioned by the authors in the discussion section. In the practical context of the study, the authors give the assumption that patients were allocated pseudo-randomization to physicians within the same hospital while not fully satisfying the causal inference in statistics but this was a reasonable, feasible, and contextually appropriate choice. However, the dangerous is that a many people criticized the study design solely for not following an RCT design, and even they went so far as to question the authors’ COI, without a proper understanding of the essence, limitations, and applicable conditions of RCTs, reflects a deeply concerning misconception. This not only undermines scientific discourse but also poses a societal risk by eroding trust in reputable scientific journals.
