@king_pearxe I had a patient like this on Tuesday. His BP was 240/150mmHg. I wanted to keep him for observation and BP control. He SAMA and his excuse was that he was anxious and did not sleep the previous day because there was no light so everywhere was hot.

I recently contacted a Neurosurgeon SR in training. I told her about my interest. She asked me two weird questions, if I was married and from a rich family. I was initially confused, then she explained that Neurosurgery will take away my entire waking moment for the entirely of training. You'd barely have time to sleep and read in the midst of work. She says she can't remember the time she went home or saw her kids. She told me to have a long talk with my wife before embarking on it in the future, as a lot of people enter only to quit halfway. For all my Neurosurgery enthusiast, I just say make I update Una.

NAIJA HOSPITAL SHEGE CHRONICLES Ep 05 Being a registrar for surgery residency in a Naija teaching hospital na like you don enter long-term relationship with stress with no break-up option. You don already survive house job, pass primaries by fire by force, and now dem don post you to your dream department, Surgery! You resume 8am sharp and by 10:15 you don already: ☑️Seen your colleague disgraced in morning review ☑️ Received 5 consults from the accident and emergency. ☑️ Reviewed 14 ward patients (including the one wey dem say “just small head injury” but was subdural hematoma) ☑️ Been called “oga reg” by house officers wey dey look at you like saviour and consultant wey dey treat you like small pikin ☑️ Written consults for 7 other departments (because every case suddenly get heart, kidney, or sugar component) You no chop since morning. Your own BP don dey rise small-small from the hypertension patients wey no dey take their drugs. You dey explain to one family for the 7th time why their father need urgent surgery when dem never pay last admission bill. Then afternoon round with consultant: “Reg, why is this patient not in the ICU ?” You explain. For your mind you want tell am say no be your papa get ICU. Consultant shake head: “No, you for think deeper.” You swallow the correction like bitter leaf soup. Evening call start at 4pm sharp. One new admission; one small boy that climbed mango and fell on his back. He can't move any of his limbs, but he is breathing fine. You're suspecting internal decapitation. If he doesn't get operated in 12 hours, he'd never walk again and your Neurosurgeon is on sabbatical. 'Isn't the lord great?,' you said in your mind. You send a message to your HOD about it. Another one: stroke patient wey family bring late, now hemiplegic and aphasic. CT shows a large bleed. Inoperable. You do your best, but inside you know wetin time fit do. By 2am, you dey write notes under dim light (NEPA never bring light since morning and gen don off since 10pm). Your eyes dey close but ward nurse still dey drop new file: “Doc, we have an acute abdomen, perforation confirmed with X-ray” You go attend, instruct one of your house officer to work up, get blood, inform theater and call you when patient is in the theater. You see four more patients. Repaired two bad lacerations from road traffic accidents, you tried giving copious analgesics but pharmacy said you're doing too much. You no argue. You put another accident victim with femoral fracture on traction and counselled them about the long and expensive surgery and recuperation ahead of them. You head to the call room hoping for 30 minutes of rest. On the way, your house officer call you that patient is on the table. You head over, buy coke and gala on the way and end up doing block resection and anastomosis. By the time you finish in 7am, you don dey feel dizzy. Accident call you for another laceration, you pass am to your second house officer. Make him use that one learn. You are already asleep before you get on bed. Morning come like thief. You no even sleep for 30 minutes. You look like person wey fight bandits all night, shirt and hair rumpled, and tie loose. Because you no get time to read overnight, they grill you like suya for 8am morning review. But you no mind. Consultants said na dust dey your brain. E no even move you. With your tired ass, you still did ward rounds, taught students and attended to consults till 4pm the next day. By 4pm, you drag yourself to car. One patient wey you manage yesterday wave from bed: “Thank you oga doctor. God go bless you.” You smile small and nod. Residency sweet for mouth, but the body and mind dey pay serious price. Nigerian doctors for training deserve better pay, better rest, better support… and consultants wey go encourage instead of only correct. We laugh through the grind… but the fatigue na real companion. I focused on my own life, my walk with God, my healing. Months passed. Then a year. One day, she texted: “I need to see you.” We met at a café. She looked different, calmer, softer, like someone who had survived a storm. She said she had been wrong. Not wrong about feelings, but about timing, fear, and assuming peace always means exit. “I thought God was removing you,” she said, “but I think He was shaping both of us.” We didn’t rush anything.
