“Trump has an alcoholic’s personality” is a perceptive statement and not at all an insult. If you look at the phenotype that performs better than anyone else in extreme situations…hunters, warriors, fighter pilots, EMTs, etc…they have the variant of the COMT gene that drives rapid clearance of excess dopamine in the prefrontal cortex. These people perform better and are calmer under pressure. But because their brains are eating dopamine 4x faster than a normal person, they are prone to boredom when the pressure is off. In a classroom, a kid with this gene looks like they have an attention deficit because they are bored. In a crisis, they turn into a high performance machine. It’s why I’ve long argued that ADHD isn’t a disease or disorder, but an important personality type without which you can’t have a society. This gene is a root cause for a lot of alcoholics because alcohol consumption initially spikes dopamine, but because the dopamine is rapidly cleared they drink more and more to chase that feeling. You see how bored Trump gets in normal mundane situations, and how lively he becomes in high stakes situations — this is why his best media appearances are always with journalists who are out to get him, or why he jumped up after getting shot to pump his fist in the air. There is an incredible amount of alpha in being a teetotaler with the wiring of an alcoholic.

I have had severe acid reflux as a result of my inability to burp, because the trapped air pressure in my stomach and esophagus pushes the lower esophogeal sphincter (LES) open and pushes stomach acid up my esophagus and into my mouth. This causes major dental issues and also makes my entire esophagus inflamed. I had an EGD, where the doctor put a camera down my esophagus to check out how it's looking. She found that my whole esophagus is inflamed, which is what you basically always see when someone has acid reflux. She wants me to go on PPIs to manage my reflux by reducing my stomach acid. I refuse to take to PPIs because 1) stomach acid is important and I don't want to lower mine, 2) PPIs are known to slow digestion (related to #1), and my digestion is too slow as is, 3) digestive problems, such as PPIs can cause, bring their own chronic health risks, which I have experienced bountifully, and 4) I am certain that when I am able to burp, my acid reflux will go away, so I'd like to avoid treating the symptoms rather than the root cause. When I explained this to her, she didn't deny my concerns. Instead, she said: "Well Tyler, your esophagus is inflamed. This will eventually lead to Barrett's esophagus and esophogeal cancer. So, if you had to pick between digestive problems and esophogeal cancer, what would you pick? She is right. But damn, what a shitty decision to have to make. There's no right answer, both answers are bad. So the real solution is to identify the root cause, which for me, is a problem with my cricopharyngeus muscle, which I working on solving. But this just shows the importance of addressing the root cause, and addressing it early, because health problems beget more health problems, and the decisions become bleaker the longer they progress.

As your health issues progress, which often happens as you get older, the health decisions you make, and the choices you have, become more severe. You often wind up having to make decisions where both options suck. For example, if your health deteriorates enough, you might have to decide between: -Getting an x-ray or CT scan to check if a cancer is causing your symptoms, or risk a potential cancer going undetected (the former especially risky if you have recent symptoms that suggest a potential cancer) -Go on dialysis, or risk the dangers of barely functioning kidneys -letting your body try to fight a cancer you have, or getting chemo (or radiotherapy) Part of the reason it's so important to take care of your health when you're young, is because if your issues progress enough, the decisions you have to make become bleaker. Of course it's always better to let your body heal itself, but if your body gets damaged enough, there are situations where medical intervention is the lesser of two evils.

@GutOptimized Any tips for increasing intracellular potassium? I’ve been getting many blood sugar headaches recently, and I suspect potassium has a role to play as back of the head and neck pain is much more exaggerated than it was a few years ago. Also potassium shows low in labs.

This charge balance is disrupted in those with pain disorders. If you're using your phone before bed, you're not reaching deep sleep. You get 8 hours of dopamine reset every night. Don’t ruin it by doomscrolling within 60 seconds of consciousness. How stimulated you get in the AM sets the pace for the rest of the day. Do meaningful stuff, slowly.
