@JAMA_current 4 years is not enough. While I see how going to college for 4 years before med school serves some students, itvadds unnecessary schooling years for folks who know early they want to be doctors. The 6 years programs prepare better doctors

@pgMedicine Hypercapnia does not cause syncope, it causes altered mental status but does not meet the definition of syncope ( sudden onset, short duration- 1 min or less, spontaneous complete recovery)

@AaronGoodman33 Good luck in your new adventure. Private practice works when it has the right setup and with the right people. As a hospitalist my experience as an independent contractor with a private group is the cautionary tale. Still recovering from it after 9 months

@Darcy_ID_doc hope all is well. Getting questions about Resolve MDX for UTI dx, cannot find much literature, mostly company ads. Is it just a faster culture? OK to use? I assume still requires symptoms etc for dx

@malkeasaad I was a board certified internist in Europe when I moved to the USA. Based in my experience, I recommend at least one year of training when moving over. Learning the system is important. Personally I am happy I went through full residency again here

@VPrasadMDMPH Plus milestone ratings are based on evaluations from faculty, which are at least partially subjective and with low completion rates

Problems:
#1 Despite efforts, patients are not randomly distributed & it's possible higher test scorers get jobs that allow them flexibility to see healthier pts
#2 instead of ABIM test, you could rank docs based on a test of US history. Higher scorers may have lower mortality bc it's not the content of the test that leads to better outcomes, in fact, that may be entirely erroneous, but the type of person whose brain is able to do well on tests, who makes better decisions. I would bet big money on this.
#3 none of this validates the abim which is a failed organization. @AaronGoodman33

@jason_ryanmd 50% is too high, as an average. But 20% , probably. The strategy is delay, rather than risk it and fail. First time pass is important for the student and the school. Residency programs would have to analyze the timeline on that application thoroughly to detect it
