Lol at this thread. I had to apply to MA, NH, and ME this year. ME easiest and NH hardest (they require criminal background check with finger printing). Sounds like there are worse stories around the country though

A patient on dialysis presents with nausea vomiting and severe chest pain. “Possible STEMI”. What’s the culprit?

Interesting AF case yesterday.1st time persAF. Following PVI/PWI, rhythm transiently organized into MA flutter periodically, suggesting that it might be a driving circuit for the AF. This led to decision to do lateral MIL. We usually balloon occlude the CS to facilitate this.

@DavidLBrownMD I would cardiovert. Worth a shot at maintaining sinus without significant risk. She’s at risk of side effects from medications and being admitted to hospital if shows up in someone’s clinic with rates in 100s

80-year old woman sent urgently to my office by her PCP for afib at 165 bpm. Already on carvedilol 12.5 and apixaban. Refuses to be admitted or seen in the ED. Asymptomatic. BP 140/90. Not in heart failure. Suggestions?

I only went out for necessities and wore masks. And did nature walks. This is obviously a very transmissible virus. People need to take a lot of care and the idea we can just reopen is very scary.

I have the coronavirus. I’ve been sick for a week, and really, really fatigued, with muscle pain. So far, thankfully, no fever and my oxygen levels are fine. But I was super careful, wore masks, carried hand sanitizer. Haven’t really socialized with friends.

@narrowQRS @EPeeps_Bot Well it looks like there’s VAWB which I suppose might indicate AVNRT less likely? The device is over sensing far field v which is why it’s reading AF

@tandonku @EPeeps_Bot I would more precisely characterize the AV relationship as "associated with mostly 1:1 relationship but occasional "missing" atrial egm," rather than say "dissociated" (see middle image). Does it start with a PVC? (first image) How does it end? (third image) What's ruled out?

@narrowQRS @EPeeps_Bot Fair looks like it starts with V pacing (though I don’t know what triggers this and terminates with V pacing). The pack of 1:1 relationship (with more V than A rules out AVRT and AT) leaving VT and AVNRT in differential
