Agree, the decision-making on who to treat and managing patient expectations is the most challenging part. What we are seeing in practice is all over the place from people treating everyone that shows up to thoughtful evaluations based on the most current understanding of who best responds. These new guidelines in theory are a step in the right direction however gatekeeping the procedure to specialist referrals is an interesting twist.

Some Pts do great and are super happy. Technically simple. Going the way of vein ablation it seems. High utilization low reimbursement.

I don’t know who reads this anymore, but am I the only one surprised @Cigna has approved GAE in addition to other MSK procedures as an approved treatment using CPT 37243? @SIRspecialists @OEISociety

@SamaHoole Initial consultation is 30 to 45 minutes. That is actual time with me. Number of papers read since med school I can’t even quantify. Thousands? Including a number of massive textbooks. See reps (mostly device) a few times a year for updates. Never been to Barcelona.

Your doctor has 15 minutes to see you, hasn't read a research paper since medical school, and gets their drug information from pharmaceutical reps who take them to conferences in Barcelona. They're not evil. They're just captured by a system that profits from chronic disease management, not cures. Questioning them isn't conspiracy. It's basic pattern recognition.

@TheRealDoctorOs As you well know this is the origin of GAE. What’s bizarre is that when bleeding is the indication, that often triggers an approval for a procedure that is otherwise denied.
