A year ago, Dr. Prasad came to the FDA to implement 4 major long-lasting reforms: 2-to-1 pivotal trial requirement, national priority reviews, a risk-stratified covid vaccine framework, & the new plausible mechanism framework for ultra rare diseases which we launched last week. Also, under his leadership, his center hit a record number of approvals in Dec. He got a tremendous amount accomplished within his one-year sabbatical from UCSF and will be returning back to his academic home later next month. We will name a successor before his departure. I want to thank him for his service and personal sacrifice to take time away from his family.

The American Medical Association represents less than 15% of practicing physicians. It controls the CPT code system that governs how every physician in America gets paid. It receives licensing fees from that system regardless of AMA membership. It lobbies on behalf of positions most physicians actively oppose. It endorsed policies that accelerated consolidation, employment, and the commoditization of physician labor for decades. The AMA doesn’t represent physicians. It licenses the billing infrastructure they’re forced to use. These are different things. The conflation is not accidental.

People realize that one way to game the heart transplant system is to implant surgical Bi-VADs (i.e. CentriMags) insetad of a durable LVAD, and thereby attain status 1 for patients. No hemodynamic criteria need to be met. I know of one program doing exactly that.

Being a hepatologist means you diagnose a LOT of heart failure 1. Ascites, often with high total protein 2. ALT > 400 is most often due to ischemic hepatitis. This is caused most by venous congestion and less by systemic hypotension. echo often unhelpful

If you cannot pass a cardiovascular board certification examination, you shouldn't be a cardiologist. FULL STOP. It's not that hard of a test. It tests BASIC knowledge. The public deserve to know their doctor is competent. STOP the dumbing down of medicine.

3 out of 7 newly diagnosed ATTR-CM patients that I saw in 1 day didnt have ATTR-CM (2 on therapy). Just really tough to interpret PYP scans with blood pooling, and due to cavity size and shape they simply appeared as myocardial uptake to those reading them. That is why we are running REVEAL (Evuzamitide in suspected cardiac amyloidosis). Cant afford making such mistakes. clinicaltrials.gov/study/NCT06788… #CardioTwitter #CVNuc
