Moderate volume clot can also be symptomatic - and doing so can significantly improve distal perfusion 25m skin to skin with filter placement in this patient with hypoxia and right heart strain, and recent GIB. PA pressures 21->11 mmHg. Lots of data upcoming, but I believe thrombectomy undoubtedly has a role in PE management

igsmissouri.com My journey is chronicled on linemonkeymd.com I did not enjoy standard IR/DR hospital based practice (2018-2020), so I left to open an OBL with a cardiologist (2020-2021). Left that practice to do several years of independent contracting with both hospitals and my friends/mentors with physician owned OBLs from 2021-2024 with the goal of creating something for myself. In the process I became very acquainted with independent IR practice models, the business of medicine and the non medical knowledge necessary for practice success. Introduced to @OEISociety and now proud to be on their board. In October 2024 I opened my outpatient practice, Image Guided Solutions of MO. We are an independent practice that is part of a larger multidisciplinary network in mid-Missouri. I take occasional weekend call at the University of Missouri where I get I to interact with IR trainees and keep up hospital skills. I have teamed up with several friends/colleagues to create Travelier and @PhysStaffSolns in an effort to make independent IR practice more accessible to our friends and colleagues. I am glad that this conversation is happening @SIRspecialists and will help how I can, but we need the society to move quickly and understand that unpopular decisions will need to be made to ensure the survival of what should be the best specialty in medicine. First we define who we are then we need to work hard to promote physician independence and autonomy. I am very worried about the future. The talent in IR is there, but the leadership has historically not been. Let’s change that.

Salary should be discussed and talked about frequently. Academic types always downplay salary and money. This ain’t 1985. Money matters a lot. More than ever
