@sarahbingo58 @Mappy6984 Good luck. I went through this a couple years ago. I had a Gleason score of 7 and decided against surgery or radiation. I went with the Tulsa procedure. It's been over 1-1/2 years and everything is good so far.

To add to this, because I’m sure a lot of people will read this and say, if she was already lean and clean eating and exercising, what could she actually have done differently? Here’s the honest answer. The first thing she could have done is know. Awareness is the intervention that precedes every other intervention. A lean, active, clean eating patient with a rising fasting insulin and unfavorable TG/HDL ratio is not doing nothing. They’re catching the problem before it catches them. Once the invisible measure is visible, the conversation changes entirely. Specific things that move the needle for this phenotype: Resistance training. Not just cardio. Skeletal muscle is the largest glucose disposal organ in the body. Lean TOFI patients often do plenty of cardio but almost no lifting. Adding two to three sessions per week of meaningful resistance work increases insulin sensitivity at the muscle level independent of any weight change. Protein intake. The clean eating demographic is often underprotein, especially women. 1.2 to 1.6 grams per kilogram of body weight supports lean mass and helps displace excess carbohydrate and refined fat from the diet without creating a deficit. Carbohydrate timing and quality. Not low carb necessarily. But moving most carbohydrate intake to around training windows, and prioritizing whole food sources that don’t spike insulin as aggressively, can substantially reduce the postprandial insulin load that drives visceral fat accumulation over time. Sleep. Chronically short or fragmented sleep drives cortisol, insulin resistance, and visceral fat independent of diet and exercise. Most TOFI patients are sleeping 5 to 6 hours and don’t think of it as a metabolic problem. It is. Stress management. Chronic sympathetic activation drives visceral fat deposition. Cortisol is the thread connecting poor sleep, overwork, and hidden metabolic dysfunction in otherwise healthy looking people. Address hidden inflammation. hsCRP elevated in a lean person usually points to something. Often gut, sometimes autoimmune, sometimes dental, sometimes chronic infection. Worth investigating rather than ignoring. Recheck the panel quarterly. The only way to know if any of this is working is to measure again. Fasting insulin should be trending toward 5 or below. HOMA-IR toward 1.0. TG/HDL toward 1 or less. ALT normalizing. If the trajectory is right, you’re winning. Can it be prevented? Yes, almost always, if caught early enough. The TOFI phenotype responds remarkably well to targeted intervention because the upstream biology isn’t broken yet. It’s just drifting. Catching it at fasting insulin of 12 is a very different problem than catching it at fasting insulin of 25 with compensatory hyperglycemia already developing. The window is real. Most people just never know they’re in it. Don’t wait for the diagnosis. Read the label.

My list is small and I’m not ashamed of it. The 4 AM alarm because the gym at 5 is the best hour of my day. The family walk every evening where my son finds a new stick and my daughter pretends she doesn’t want to be there but hasn’t looked at her phone in twenty minutes. Saturday morning coffee before anyone else is awake. And the quarterly blood panel because I’m the kind of person who gets excited about a fasting insulin result the way normal people get excited about vacation photos. Small things. Repeated consistently. That’s the whole protocol for everything. And a family vacation. Gotta always have one of those planned at all times to look forward to also 😃

I just went through this myself a couple months ago. Primary care physician ordered blood tests which subsequently showed I had a high creatinine level and my eGFR was low.  I had explained I eat a high protein diet, consume creatine and resistance train.  She wasn't aware of the Cystatin C blood test.  Per her suggestion I visited with a kidney doctor and her ordered this test and everything came back fine.  Kidney doc said he sees a couple people a month like this.

Great question and you’re right to be skeptical of the standard kidney markers in this context. This is my lane so let me untangle it. Creatine and creatinine are directly related molecules. Your body naturally converts creatine to creatinine at a relatively constant rate. Creatinine is then filtered by the kidneys and excreted. The standard eGFR calculation uses serum creatinine as a proxy for kidney filtration. Higher creatinine, lower calculated eGFR, which gets interpreted as reduced kidney function. Here’s the problem. When you supplement creatine, you increase the total creatine pool in your body. More creatine means more creatinine produced as a normal metabolic byproduct. Your serum creatinine goes up. Not because your kidneys are failing. Because there’s more substrate being converted. The eGFR calculation sees the elevated creatinine and spits out a lower number that looks like kidney impairment on paper. But the kidneys are doing exactly what they’ve always been doing. They just have more creatinine to filter because you gave them more creatine to work with. At 10g daily this effect is even more pronounced. You could have a creatinine-based eGFR that technically flags as reduced and trigger a conversation with your doctor about kidney disease when your kidneys are perfectly fine. I’ve seen this happen. It’s happened to me even. Healthy guy supplementing creatine gets blood work, eGFR comes back at 72, doctor panics, patient panics, nobody connects it to the 10 grams of creatine he takes every morning. The solution is Cystatin C. Cystatin C is a small protein produced by all nucleated cells at a relatively constant rate. It’s filtered by the kidneys the same way creatinine is but its production is completely independent of muscle mass, diet, and creatine supplementation. A Cystatin C-based eGFR gives you an accurate picture of kidney filtration that isn’t confounded by your supplement protocol. If you’re taking creatine at any dose and you want to monitor kidney function honestly, ask for Cystatin C. Most reference labs run it. It’s not part of standard panels but any physician can order it. Run it alongside the standard creatinine-based eGFR. If your Cystatin C eGFR is normal while your creatinine-based eGFR looks low, your kidneys are fine. The creatinine is elevated because you’re supplementing. Not because your kidneys are struggling. As for cycling off. The evidence doesn’t support the need for it. Creatine monohydrate has been studied for decades in continuous dosing without demonstrated kidney harm in healthy individuals. The “take a week off” advice comes from the bodybuilding forum era, not from clinical data. If your Cystatin C eGFR is normal, there’s no physiological reason to cycle. At 10g versus 5g. The muscle saturation data suggests 3 to 5 grams daily is sufficient for most people to maintain full intramuscular creatine stores after an initial loading period. 10g isn’t harmful but the extra 5g is likely being excreted rather than stored. Which means you’re paying for creatinine production, not additional benefit. The cognitive data from the sleep deprivation study used 32g in a single dose (for a 200lb male) to force central uptake acutely. 10g daily is a middle ground that probably isn’t doing much more than 5g for either muscle or brain but also isn’t causing harm. My recommendation. Stay at 5g. Order a Cystatin C alongside your next metabolic panel. If it comes back normal, you’ve confirmed your kidneys are fine and you can stop worrying about it. If you want to stay at 10g, same test, same answer. Let the data decide instead of the forum. Don’t wait for the diagnosis. Read the label.

Once the numbers are available, it is possible to run an analysis to identify the fifty worst judges in America, meaning those who are putting the country and its citizens in severe danger. You go after those first fifty. You use whatever legal means are available to get them impeached. You investigate whether they have engaged in corruption. Whatever is within the scope of the law, you use to remove them from power. As people are removed from the list, you replace them with the next worst remaining, so the list always contains fifty. If you were to do this with a couple of wealthy sponsors, you would likely need a team of three or four people to handle the investigations and a lawyer to file suits. That effort would provide a significant service to America and Americans. Put the judiciary on notice.
