Vor kurzem habe ich Euch erzählt, dass ich @MobileRetter installiert habe. Gerade ging ein Alarm auf meinem Handy los: Reanimation in 400m Entfernung. Wir waren zu zweit und haben mit der Reanimation begonnen bevor der Rettungsdienst da war. Etwa 4 Minuten Zeitgewinn.

@dmtecc @Herzspitze @HDstabil Also für mich würd ich das so zsmfassen: Hypothyr.: TSH sensitiver und eig Hauptparameter zur Beurteilung, benötigt aber bei Auffälligkeiten und best. Sit. fT4 zur umfassenden Beurteilung. fT3 eher für Hyperthy. relevant.

@dmtecc @Herzspitze @HDstabil Lothar Thomas sagt dazu in Labor und Diagnose 2020: Bei amb Pat und Klinikpat. ohne schwere Allg. erkr. kann SDstatus akkurat durch die Bestimmung des TSH im Serum festgestellt werden. Zustzl Bestimmg. zur Beurteilung der SD Fkt wie FT4 sind nur erforderl., wenn TSH abnormal ist

@Herzspitze @HDstabil deswegen ja bei stabiler Situation. Was nimmst du denn sonst zur Anpassung -fT4?

@Greggi18 @HDstabil Je nach Präparat ist der TSH unter Therapie halt überhaupt nicht verwertbar. Z.B. für Henning bekannt. Supprimierter TSH trotz niedrigem fT4 (und fT3). Was am raschen Anfluten liegen könnte.

@HDstabil Ähm. Ich hoffe, Du hast vor Allem die Freien noch bestimmt und eruiert, wie lange L-Thyroxin was/warum. Und Risikofaktoren angeschaut. Nach dem TSH stellt man grundsätzlich nicht die Hormontherapie um.
