Doctors at the University of California, San Francisco witnessed an extraordinarily rare medical event when a 36-year-old man coughed up a complete blood clot in the shape of his right lung’s bronchial tree — the intricate branching airways inside the lung. The patient, who had severe chronic heart failure and was on a ventricular assist device, was taking blood thinners to prevent clotting. While these drugs reduce clot risk, they also increase the chance of bleeding. After several minor episodes of coughing up blood, the man experienced a violent coughing fit that expelled the intact, branching cast of his airways, formed entirely from clotted blood. Such casts are occasionally made of mucus or lymph, but blood — being less cohesive — almost never retains such perfect anatomical structure. In this case, an infection likely boosted the patient’s fibrinogen levels, a protein that helps stabilize blood clots, allowing the cast to hold together. While the clotting episode itself did not recur, the patient sadly died a week later from complications of heart failure. Doctors published the case to highlight both the rarity of the event and the remarkable detail it revealed of the lung’s anatomy.
