Metastatic Merkel cell carcinoma in the CSF – pt w/ a known h/o the disease and suspicion for leptomeningeal carcinomatosis by imaging. Cytology shows loosely cohesive clusters of round cells w/ high N:C ratio and characteristic finely stippled, powdery neuroendocrine chromatin.

CC383. Some diagnoses are like fruit, they need time to mature. Here is an example. Elderly patient with a history of colon Ca. presents with peculiar mesenteric lesions (change in size and shape). Metastasis? Infection? FNA images below. Flow cytometry shows a reactive pattern

It is rare in the tubular gastrointestinal tract, but globular amyloid is associated with light chain restriction. There is no neoplasm here, but the patient should be investigated for a plasma cell disorder. PMID: 17197930.

Super scary. At low magnification, this little focus looked like "squamous morules" to me... boring, next case..... but the cells are ugly and mitotically active. A tiny neuroendocrine carcinoma arising in the setting of a tubular adenoma. Gulp.

@ananthvikas Yes. But I’d add: the median monthly income in India is ~$325. So while $500 for full testing may seem accessible, it’s still out of reach for many. And the bigger question remains—will it change outcomes?

I’m genuinely glad the Hempath, WHO, and ICC groups are getting along now. Kissing and making up is great. But here’s a cold, hard truth: I’d bet that in your 7th classification, the vast majority of what you include will be inaccessible to 80% of the world’s population. Let that sink in.
Vikram Deshpande@Vik_deshpandeMD

Controversial but crucial: The new WHO Blue Books heavily rely on molecular diagnostics, tools inaccessible to up to 80% of the world’s population. Instead of addressing global inequality, they risk deepening it. Radical, inclusive change is urgently needed.

For suboptimal biopsies for which a diagnosis of oxyntic gland adenoma is difficult, cyclinD1 can be helpful! PMID: 39863113.#UMiamiPath @ModernPathology
