Did you use Chiba needle through TIPS metal cannula or manually bending the Chiba needle to get the required curvature? If ascites could be drained (under Albumin cover), Percutaneous Transhepatic would be possible as stent is easier to visualise under USG.

39 yr/M Budd Chiari Syndrome, s/p IVC stenting 2004 - Wallstent which migrated into RA which was occluded and then re-stented using Eluminex in 2021. Patient had recurrent ascites despite recanalizing IVC. Performed DIPSS through these stents. Currently ascites free!
