I belong to a generation where, as a girl MBBS student, I ate chicken biryani last night and went to the General Medicine OPD with abdominal pain and loose stools. When I consulted my Medicine HOD, instead of treating my condition, she asked me to do a UPT first.

📌BREAST FEEDING AS A CONTRACEPTIVE After giving birth, when a woman is breast feeding 🤱, prolactin hormone is produced inside her, and this inhibits FSH and GnRH hormones, which would’ve triggered ovulation and maturation of her eggs, hence preventing pregnancy during this time. It’s 98% effective(as effective as birth control pills) It’s most effective during the first 6months post delivery.

➰Paracetamol vs Mefenamic Acid in Children — When to Choose What? ♨️♨️Paracetamol (Acetaminophen) — FIRST-LINE choice 🤹Best used when: •🌀Simple fever (viral fever, post-vaccination) •🌀Mild–moderate pain (teething, headache, earache) •🌀Infants & young children •🌀Child has gastritis, dehydration, asthma, kidney risk 🤹Why preferred? •🌀Acts centrally (COX inhibition in CNS) •🌀Excellent safety profile •🌀No gastric irritation, no platelet inhibition 🤹Dose:- 🌀10–15 mg/kg/dose every 6 hours 🌀Max 60 mg/kg/day ♨️♨️Mefenamic Acid — SELECTIVE / SECOND-LINE 🤹Better option when: •🌀High fever with inflammatory component •🌀Moderate pain not responding to paracetamol •🌀Dental pain, musculoskeletal pain •🌀Older children (usually >6 months–1 year, preferably >5 yrs) 🤹Why more potent? •🌀NSAID → peripheral COX inhibition •🌀Stronger anti-inflammatory + analgesic effect 🤹BUT be cautious:- •🪬Gastric irritation, vomiting, diarrhea •🪬Renal risk (↑ with dehydration) •🪬Not for routine/simple viral fever •🪬Avoid in infants, dehydration, renal disease 🤹Dose: 🌀4–5 mg/kg/dose every 8 hours 🌀Short duration only 🤹CLINICAL PEARL:- ♨️Paracetamol = SAFE default ♨️Mefenamic acid = STRONG but selective 🪬Never combine NSAIDs unnecessarily 🌀Treat fever for comfort, not the number 🤹Bottom Line:- 🌀Paracetamol first, always 🌀 Mefenamic acid only when clearly indicated #Pediatrics #Pharmacology #FeverManagement #NSAIDs #Paracetamol #MefenamicAcid #ClinicalPearls #KriMeeraPharma

Vonoprazan is a potassium-competitive acid blocker (P-CAB) that directly and reversibly blocks the gastric H⁺/K⁺-ATPase. Unlike PPIs (pantoprazole, omeprazole, esomeprazole, rabeprazole), it does NOT need acid activation - meaning Day-1 efficacy, food-independent dosing, and predictable response 👌 What makes it better than PPIs? • ⚡ Faster onset (hours vs 3–5 days with PPIs) • 🌙 Superior nocturnal acid control • 📈 Stronger & sustained pH >4 control • 🧬 Minimal CYP2C19 dependence (key in Indian population) • 🍽️ No “take before meals” headache Clinical indications where Vonoprazan shines ✨ • Refractory GERD despite adequate PPI trial • Severe erosive esophagitis (LA grade C/D) • Prominent nocturnal reflux symptoms • Poor compliance with PPI timing • H. pylori eradication, especially after PPI failure Dosage (Adults) 💊 • GERD / erosive esophagitis: 10–20 mg once daily • Maintenance therapy: 10 mg once daily • H. pylori eradication: 20 mg once daily (with antibiotics) ➡️ Can be taken anytime, with or without food
