@DrDiGiorgio Just got my bill for annual PCP visit. I have the highest deductible plan my employer allows. The PCP would probably happily take $300 cash at the door, I would have a lower premium, and everyone would be better off except for the middlemen.

This is a terrible idea. Free primary care for all sounds great until you remember that free never means free. It means the bill moves from the exam room to taxes and new bureaucracy hired to ration what politicians just promised was unlimited. If car insurance covered tires at zero cost, demand would explode. People who truly needed tires would wait behind people getting upgrades because why not, it is free. Then the government would demand forms, approvals, documentation, denials, appeals, and entire departments to manage the mess. More primary care visits do not automatically mean better health just as free tires don’t reduce car accidents. More spending does not automatically mean better care. And making primary care “free” does not make doctors, nurses, clinic space, time, or judgment magically materialize out of the ether. It just removes price signals, politicizes what counts as essential, invites every interest group to lobby for inclusion, and leaves patients and physicians trapped under another layer of central planning. If you want universal access for primary care, which I do, then just people the money. Primary care visits are cheap and the median American will spend less on those than on food. We have food stamps for the latter, so let’s make a type of food stamps for healthcare. Let patients and doctors decide what care is actually worth it for that individual instead of creating yet another government promise that doesn’t work out.

An under discussed topic of our time is that having a larger percentage of adults be childless alters our culture. Fertility decline doesn’t just equal less babies, it translates into less empathy… a coarser culture.

Growing up in Colorado, I watched the state fundamentally change its culture after marijuana was legalized in 2012. It is utterly unrecognizable today, and is not a place I would currently feel comfortable raising my daughter. From 2013 to 2021 in Colorado: • Traffic deaths where drivers tested positive for marijuana increased 138%. All traffic deaths increased 29%. • Past month marijuana use (ages 12 and older) increased 26% and was 61% higher than the national average. • Youth marijuana use (ages 12–17) was 39% higher than the national average. • Marijuana-only exposures increased 185%. • Suicide incidents in which toxicology results were positive for marijuana increased from 14% to 29%. Marijuana legalization was promised to be a cash cow for the Colorado public schools, finally fueling more tax revenue into our kids’ education. But in FY 2020, Marijuana tax revenue represented only 0.98% of Colorado’s budget. Nothing about this is showing a state setting the standard for American excellence - it shows a culture leading the race to the bottom.

Very difficult to maintain a building when there are no viable tenants, but this is one of the great acts of architectural vandalism in the US. St. Louis is a magnificent city. It tore itself down. We may never see these buildings rise again, but if we're luck, we can save what treasures do exist from meeting the same fate of these buildings (razed for parking)

Though this site is virtually unique in STL in that its incredibly spiritually active with ~600 families and growing. The neighborhood is also on the rise with numerous commercial, non profit, and residential advancements in the last 5-10 years.
