@EricTopol Can this be related to some heterogeneity of treatment effect? For e.g. I remember previous trials estimating effect for obesity alone recruiting more women than men. But sadly I have not access to the Table 1 of this new trial to investigate such hypothesis.

No one knows why people who are obese with Type 2 diabetes lose much less weight on GLP-1 drugs compared with obesity alone. These 2 trials testing the same dose and drug really bring that mystery point home.

@f2harrell's paper fharrell.com/post/rct-mimic…, explains why RCT are not falsifiable, "Randomized clinical trials are successful because they do not mimic clinical practice", namely, because practitioners refuse to bow to any external criterion of performance - it must be internal.

RCT studies are not falsifiable because RCT practitioners reject the idea that RCT has a goal, named ATE, residing outside the RCT protocol. Once they accept ATE=E[Y(1,u)-Y(0,u)], RCT results turn falsifiable, see Eq.(9.32-9.33) in ucla.in/3akkcbA. Sadly they reject it.
