I'm lucky enough to have a great doctor and access to excellent Bay Area medical care. I've taken lots of standard screening tests over the years and have tried lots of "health tech" devices and tools. With all this said, by far the most useful preventative medical advice that I've ever received has come from unleashing coding agents on my genome, having them investigate my specific mutations, and having them recommend specific follow-on tests and treatments. Population averages are population averages, but we ourselves are not averages. For example, it turns out that I probably have a 30x(!) higher-than-average predisposition to melanoma. Fortunately, there are both specific supplements that help counteract the particular mutations I have, and of course I can significantly dial up my screening frequency. So, this is very useful to know. I don't know exactly how much the analysis cost, but probably less than $100. Sequencing my genome cost a few hundred dollars. (One often sees papers and articles claiming that models aren't very good at medical reasoning. These analyses are usually based on employing several-year-old models, which is a kind of ludicrous malpractice. It is true that you still have to carefully monitor the agents' reasoning, and they do on occasion jump to conclusions or skip steps, requiring some nudging and re-steering. But, overall, they are almost literally infinitely better for this kind of work than what one can otherwise obtain today.) There are still lots of questions about how this will diffuse and get adopted, but it seems very clear that medical practice is about to improve enormously. Exciting times!

@_rotimia Went to a fancy med school in nyc. All the black students in my class were wealthy African immigrants and only one who grew up in South Bronx. Huge difference in class experience.American doesnt think in class terms.

When you look at top schools and see some affirmative action spots going to wealthy African immigrants, it should give us pause—not because diversity is wrong, but because class-blind and race only policies can miss the people who actually need the help most.

There are some affirmative action and even DEI programs that are far too broad and really at the end of the day just advance wealthy people of color, that do not need the upper hand. Example laid out below in the video even proves it. Even Obama has talked about this before.

@mcuban I’m glad they are getting sued. The amount of peer to peer I do with United healthcare where they decide patient should have been hospitalized but only met criteria for observation level care. Oh and based on what criteria? Optum criteria. Who owns Optum? United healthcare.

Another peer-to-peer call. The reviewer was kind. Really kind. But, respectfully, not my peer. He listened as I explained why my patient needs lymphovenous bypass surgery. It’s a surgery that can treat the painful swelling in her arm caused by breast cancer treatment and prevent it from getting worse. She has done everything right. Compression. Therapy. Expensive garments. She’s changed her life to spend an hour a day with every resource she can afford. It’s affecting her ability to work as a nurse. And she is getting worse. This is the window where we can fix it. Early enough that surgery can actually change the course. We can do this surgery outpatient. Two hours. CMS even has a code for it. The man on the phone agreed it would help her… But he couldn’t approve it… Because he’s not a plastic surgeon and doesn’t have the specific knowledge necessary to make this recommendation. He gave me the help he felt he could. He told me to appeal and told me not to expedite it, because that wouldn’t work in my favor. Every human involved in this case knows what we should do. But the system is inhumane.

yeah I'm sick of you guys acting like if we had another manager on the sideline Sesko would finish his chances, Cunha would release the ball quicker, Fletcher would play a forward pass, and Dorgu would've been onside Amorim isn't the best and he could get sacked, but watch how quick the same narrative will happen with the next manager. The problem is many beleive we geniuly would play like 2010 Barca with today's squad under another manager. This attitude towards the manager has been same for the last 12 years. that's my issue, I'm sick of that cycle and I'm willing to trust something for longer than 2 years for the first time in over a decade. yet I'm the crazy one , the toxic one, and the problem with the fan base. apologies, let's just keep sacking then 🙏
