@DrAndrewLoblaw Biopsy to prove recurrence, focal HDR with 6 months ADT. Rectal and urethral dosevusually not an issue when treating the SV.

Case of the day:
Now 60 yo male, high risk prostate ca dx in 2017. Treated with 5fx SBRT (49Gy to DIL, 35Gy to prostate, 25Gy to pelvis).
PSA nadir 0.09; relapse 2.3. PSMA PET shows L SV involvement only

Clinical trial protocols are often useful as well. Taking part in clinical trials also can be beneficial - they often have an external review of contouring

@DrAJWalker No straight answer also could depend on the prevalence within the local population. False neg may mean false sense of security, false pos delaying trt Plus xrt lasts upto 8wks, is 1 swab enough? Currently swabbing before brachy (along with cocooning). May start some swabs pre CRT
