We’ve had two fantastic days at this year’s Glasgow Surgical Forum, giving an essential update for surgeons on preventing complications, managing them effectively and discussing best practices. Many thanks to everyone who joined us including speakers and session chairs.

Most people have no idea how bad the UK doctor training system really is. It demands double the training time of other countries, pays doctors less at every stage, and then often keeps them “junior” even after they finish all their training. Look at the numbers. 🧵⬇️

The new ESVS Vascular Trauma Guidelines 2025 have just been published, open access. ejves.com/article/S1078-… A massive piece of work, to bring vascular trauma kicking and screaming into the 21st century. Your grandpa vascular surgeon ain't going to like them!

TRAUMA LAPAROTOMY. Know it well! Standard approach: ✔️ Stem to stern (xiphoid to pubis) ✔️ Take down falciform ligament ✔️ Pack all 4 quadrants ✔️ Explore systematically ✔️ Control bleeding and contamination ✔️ Decide: Damage control? 🧵 1/3

It’s specialty interview season so I’m just reminding you all that charging people for interview prep courses is extortion not entrepreneurship.

For the attention of any doctors in the early stage of their career interested in surgery: this lecture series seems promising. I am in no way biaised having personally delivered the vascular session. rcsed.ac.uk/professional-s…

@Davemademedoit @DrLKVaughan And in some units, often written last minute or retrospectively by an inexperienced colleague that has never met the patient. I used to have to write 10+ daily on a weekend shift for a short stay medicine ward.

@DrLKVaughan I strongly dislike the reduction of discharge summaries as ‘admin and paperwork’ that ‘anyone’ can do. Actually most discharge summaries are written badly. They are a key resource for clinical information not just for GPs but patients and future colleagues.

One of the things I like LEAST about the PA debate is the construction of 'ward work' as somehow being the WORST work. Stuff that it is somehow not really the job of a doctor. How can looking after the sickest pts be considered the least skilled work?

I queried the role & spoke to the lead consultant who told me PAs will do ward work/ward procedures & Discharges. The PA project that exists today is blatantly not that & has not had the consent of the UK medical profession

@dr_woodall I don't think this should be on letters or public facing. I get the doctor is proud of their achievement. However, I see it as binary and could come across as disingenuous.
