Goals achieved: 1. Iranian Navy eliminated. 2. Air Force eliminated. 3. 48 of the 50 top evil leqders dead. 4. Less than 80 launchers remaining w a shelf life of one fire 5/ No capability to enrich an nuclear material probably for a decade (inputs destroyed like water plant, steel and most sights). 6. Very military ballistic missles supply left w no ability to produce more. 7. Some drones remain w negligible ability manufacture new. 8. Unified Gulf States against Iran except Oman 9. Chinese weapons proven to be dysfunctional. 10. Middle Eastern states lining up for business relations w Israel.

@julesyoo FFS codes are generally time-based, and time spent by an AI doesn’t count. Lower costs don’t benefit providers if they lead to even lower revenue.

CMMI ACCESS is bringing sexy back to Medicare 🎸. A lot of investors and companies pulled back from Medicare over the last few years in light of regulatory headwinds and recent performance degradation amongst publicly-traded MA businesses... and the current wave of AI care apps, while exciting, is primarily targeting younger, healthier, commercially insured users. But I've now heard of numerous startups viewing ACCESS as a reason to consider serving Medicare populations for the first time. So this program could re-accelerate the deployment novel care models into senior populations who face some of the worst access and health issues in our society! Also - a common reaction to the announcement of ACCESS rates was, "they're so low!" But that's the point! Constraint breeds ingenuity. Great conversation with @AbeSutton (CMMI) and Rohit Chandra (@ClevelandClinic) at ViVE on these and other topics related to payment model innovation in the age of abundant, AI-powered care. One debate that we didn't fully get to resolve on stage: Is FFS really all that bad? What would need to be true for FFS to be viewed as productive versus destructive? Not all healthcare interactions and customers are conducive to capitation or subscription models... and AI holds the promise of being able to deliver services (both cognitive and administrative) at a fraction of the human version of the cost, with less errors and waste. What say you?

@chrissyfarr How would you define these categories, if not minority vs. majority investing? I suspect that “PE” is being used as shorthand for “late stage investor comfortable with healthcare growth rates,” whereas “VC” now means “100x target-return AI investor” (seemingly everyone else).

I've noticed that PE and VC are getting lumped together by healthcare industry folks that I know as if they're the same thing. I'm not sure how we solve this problem but I suspect having more transparent conversations about it would help.

Within 45 days, these officers will identify which city services deliver the strongest results for New Yorkers—and where we can streamline processes and cut waste. The goal is simple: make sure every public dollar works harder to deliver a safer, cleaner, and more affordable city.

Honest question for the void. Lots of debate about whether shot #1 was self-defense, but it seems straightforward that the final shots were not (through side window from a distance). So why is shot #1 worth debating? If nothing else, innocent at t=0, guilty at t=1, right?

@zachweinberg Credible threat of U.S. military intervention seems like a more effective deterrent, but normalizing invasions sends the opposite signal. And if you’re saying that war with the U.S. wouldn’t have stopped them anyway, why won’t they also go to war for the oil required?

Unless actual healthcare costs decline, then Medicaid cuts will just get financed via higher healthcare prices (hospital bills and premiums) instead of taxes…by the same people. And if you deny preventive care and add administrative costs, total spending probably goes up.

@nikillinit I think #2 is the big one. FFS inherently pays for time spent by humans, just as lawyers get paid for billable hours. If you lower costs by reducing labor inputs, you also lower revenue. Unless you have VBC contracts, efficiency bets will be aimed at (non-billable) admin work.

It's pretty clear that healthcare costs are going to go up in the near term with the introduction of AI Three reasons for this. The first is simple - payers are being much more heavily scrutinized for their use of AI in preventing $ going out, while hospitals/companies using AI to bill more have a ton of leeway. The second is that the current billing rails incentivizes AI use cases that are "add-on" billing line items. Consider the AI second reads of mammograms which are an up charge, the AI diabetic retinopathy screens that bill more than their human counterparts, etc. which are all effectively upcharges. The third is that more people now have access to a self-diagnosing and triage tool, but also still need to go to the health system to resolve their issue if they need confirmatory labs, screening a prescription, etc. That's going to create more utilization inherently, though how much more than just using Google is TBD. -- Every time technology has been introduced into healthcare it's caused costs to be inflationary. My hope is that AI becomes a tool that makes costs deflationary, but there needs to be a clearer path for that to happen.

Today after 12+ years at Y Combinator I’ll be transitioning to a Partner Emeritus position. This role allows me to continue to do office hours with the 1000+ companies I’ve worked with in the past decade while giving me the free time to explore new adventures. It also means that the w25 batch was my last batch funding new YC companies. The next adventure I’m excited to pursue (after taking the summer to relax) is how I can help government better serve its citizens. Thank you to the countless friends who have been pushing me in this direction for years. Government was the passion of my youth and I’m excited to reengage.
