Our latest meta-analysis, by @Rowan_Burns1 et al. in BJS Open, dives into the risk factors of bile duct injury (BDI) during cholecystectomy. We analysed 31 studies, covering 6.5 million cholecystectomies and 18,259 BDIs. Key findings: ⚠️ Male sex — adjusted OR 1.27 (95% CI 1.13–1.39). ⚠️ Acute cholecystitis — adjusted OR 1.74 (95% CI 1.27–2.39). 🛑 Routine IOC — adjusted OR 0.92 (95% CI 0.70–1.23), no clear protective effect. 🛑 Critical View of Safety — although widely adopted, there is limited evidence, as often poorly documented. Take-home: ✅ Risk stratification before cholecystectomy supports better consent & planning. Our meta-analysis shows male sex and acute cholecystitis are associated with increased BDI risk — informing patient discussions, resource allocation & ensuring appropriate surgical expertise. ✅ IOC is not supported as a reliable prevention strategy for BDI. It may help confirm anatomy or detect injury, but strong evidence for routine use is lacking. ✅ Human factors — visual perception errors, stress, and fatigue — can contribute to misidentification of biliary anatomy and increase BDI risk.
