@AdamBaer88 @kpharmd12 They admit to paying at least 50% rebate on GLP-1. That is the PBM extorting money out of the system they are being paid to control costs. Patient pays huge deductible on initial fills and PBM pays nothing and collects 500 for the privilege of a pharmacy doing that work.

CVS unveils “pharmacy-only” store in Chicago. “Each location will average less than 5,000 square feet, and will feature a full-service pharmacy with limited over-the-counter products available for purchase” You just described what an independent pharmacy does. This is not earth-breaking news. Maybe if reimbursements from CVS Caremark weren’t dog-shit garbage, then more pharmacies like this wouldn’t have been forced to close in their communities. Just some food for thought @CVSHealth @cvspharmacy, not like you all give a shit about what’s best for any given community.

@kpharmd12 @Pharmassists Same story with GLP-1. Pharmacy buys it for 1k and gets paid 980 for it But you can buy direct from manufacturer or with a coupon for $199. We are running pharmacies out of business and ripping off patients with deductibles Nobody can afford half of America on GLP-1

Someone explain this to me like I’m 5 years old. If I have someone who is on Medicare Part D and needs an Advair HFA inhaler- their copay is set at $387, which results a $44 LOSS on the drug to the pharmacy. Yet, if you have no insurance coverage: you can sign up for FREE on the manufacturer’s website for a help card and get it for $35.00. The pharmacy is paid $29.00 over their actual cost of the drug. Same drug, filled at the same pharmacy. Two completely different outcomes for the patient and the pharmacy.

Just found out today Humana now only covers brand Ventolin instead of a much cheaper Albuterol. Feel bad for the patients that can't breath but I'm not filling that at a loss. Guess they will have to wait for mail order. Yay for more ER visits!

@MarketN45894046 @CSexton25 @realDonaldTrump PBM’s getting 50% rebate on most brands dispensed is only helping themselves. Watching customers pay huge copays only for the pbm to profit on each claim. We are talking $500 to the pbm on each GLP-1 script. Pt pays that because most pts now have huge deductible.

New IQVIA data shows how PBMs steer specialty prescriptions to their own pharmacies. For patients starting a new brand-name drug: • Oral oncology: 84% approval at PBM-affiliated pharmacies vs. 70% at unaffiliated pharmacies • Autoimmune: 63% vs. 61% But the pathway matters: • First-fill approvals are far higher at PBM-affiliated pharmacies (42% vs. 14% in oncology; 29% vs. 8% in autoimmune) • At unaffiliated pharmacies, many patients only get approved after switching (14% oncology; 20% autoimmune) The result: PBM-owned pharmacies capture a disproportionate share of specialty volume. A 30 day supply would be a $35 copay as mandated by the government. So even if you tried to split this up and say do 2.5 pens it would be $70 for each drug instead of $50 The retail costs of these drugs are each around $500. And with goodrx lantus would be around $350 and the lispro $150. The insurance company IS saving them money.

Have a patient with a Medicare Advantage plan… Needs both Lantus Solostar and Insulin lispro pens. Based on the dosages they use, each box of 5 pens will last 60 days. Their copay’s for each drug are $50.00 Just got off the phone with the insurance company wondering if there was any possible way to bring down the cost for them. We were asked to bill for a 30-day supply instead of 60-day. Can’t do it- manufacturer label states for single patient use only- so cannot break package. We were then asked to set up a payment plan for the patient. Also No- our PBM contract states all copays must be collected at the time of service and paid in full. Idk- I feel like the entire phone call was a setup for me to admit to committing fraud. The insurance company representative should have already known the answers to those questions.

If you have Medicare and love your independent pharmacy, switch to AARP. If you have any other plan, you're putting the pharmacy you love out of business. If you want to stick with the plan you have, you might as well go to CVS.

@ftc @linakhanFTC Independent Pharmacies in the US are paid by pbms for rx’s. Pbms own pharmacies. Indy pharmacies compete with these pharmacies. Our competition (pbms) are setting our price, and the price is intentionally below our cost. Isn’t this illegal?
